Glaucoma Provider Checklist

Glaucoma care is a complex matter. Choose a practice that focuses on safety, diagnostic technology, and if surgery is needed, thorough pre-operative and post-operative follow-up care. Obviously, the experience of the surgeon and staff is extremely important as well. You may wish to use this checklist to help make the decision that is best for you.

Does the practice have the latest visual field technology such as the Humphrey visual field test with SITA, and SITA SWAP?	Yes	No
Do doctors offer the latest, high-tech optic nerve imaging including the Zeiss OCT , Zeiss GDX and digital photographs?	Yes	No
Do Surgeons offer the latest glaucoma surgical treatment, including the Ex-PRESS™ Mini Glaucoma Shunt, filter surgery with Mitomycin , 5 FU, Ahmed valve implantation , Baerveldt valve implantation , SLT laser?	Yes	No
Does the practice have on-site lasers for glaucoma treatment including SLT, and ALT?	Yes	No
Has the practice been involved with research in glaucoma treatment?	Yes	No
Is a board certified, fellowship trained glaucoma specialist readily available for consultation?	Yes	No
Is there an office located close to you for ongoing care and follow-up?	Yes	No
Does the practice provide printed and multimedia information to help inform you about your condition?	Yes	No
