Your Appointment

Thank you for choosing Michigan Eye Institute. We look forward to welcoming you and your family to MEI. Appointments are available Monday through Friday from 7:30 am – 5:30pm, depending on office location. The Oxford office is also open Saturday’s 9 am – 1 pm.

New Patients

New patients are always welcome at Michigan Eye Institute.

To save you valuable time when you arrive for your appointment, you may complete patient information forms ahead of time.  We will mail them to you, or you can download the forms.  We ask that you fill them out return them to us before your appointment via email to patientforms@mieye.com, fax them to 810-733-7141, or mail them to our main office. 

If you are transferring from another eye doctor, we will help you have your records transferred to our office.  Click –here- for a medical record release form that you can send to your doctor to authorize release of records to us.

Preparing for Your Visit at MEI

Your comprehensive eye examination and required testing is usually complete in 60-90 minutes depending on your specific eye care needs. Glaucoma Consultation with special testing is usually complete in 2 hours. Post-operative care examinations typically take about 15- 30 minutes.

Your exam may require that your eyes be dilated. For your comfort and safety, we recommend you bring someone with you to drive you home after your visit. Special reversal drops and A full report of all findings will be sent to your referring doctor following your examination.

Please bring the following information with you to every appointment:

• A complete list of current medications, including your dosage and how often you take each medication. 
• All current eyewear including your eyeglasses, contact lenses and your latest eyewear prescription. 
• All insurance plan cards including Medicare and private pay coverage. 
• A referral form from your primary care physician if required by your insurance carrier.

All insurance co-payments are due on the day of your visit. For your convenience, we accept cash, personal checks, VISA, MasterCard, Discover and American Express.
